Information for Parents

Cost of student health care. Student health care is funded in part through privilege fees paid upon enrollment. Visits to the health care provider are "free" to the user. Ancillary Services (Pharmacy, Laboratory, Physical Therapy, Allergy & Immunization, X-ray, Dietary Counseling) are charged to the student at a reduced rate. These ancillary services (except Pharmacy) can be billed to your health insurance carrier.

Note: Students must sign an Authorization for disclosure of protected health information [pdf] in order for health information to be released to parents/guardians. (This includes discussing information with the health care provider.)

Allergy injections:

The health center administers allergy injections at regularly scheduled times. Your physician must supply written, detailed instructions. Contact the Allergy and Immunization Clinic for further information.

Appointments:

Students are seen by appointment only. This has proven to reduce the waiting time to see a health care provider. If there are no appointments available and the student needs to be seen that day due to illness or injury, a walk-in service is available.

Billing for services received:

Your student will be billed (by mail) for ancillary* health care services. These bills will not be mailed to parents. If you have questions regarding billing, first have your student call 785.532.7759 and give permission to release this information to you.

Health Insurance Information:

If your student NEEDS health insurance:
KSU offers health insurance for students at a reduced cost through the Statewide Student Insurance Program.

If your student IS COVERED by your policy:
Each health insurance policy has various guidelines regarding reimbursement and payment for health care. It is best to call your health insurance provider to obtain this information. Lafene's Business Office can submit commercial health insurance claims provided with complete and current medical insurance information, providing the insurance company has a USA address. However, there may also be certain contractual reimbursement systems with which the Health Center does not have a payment arrangement. Check with the Health Center's insurance personnel for more information.

Medical (Health) history form:

The Medical (Health) history form must be completed and signed by the student prior to the student receiving services. Students under the age of 18 must have parental permission to obtain health care at Lafene Health Center. This may be accomplished by signing the health history form. This form is available at New Student Orientation, at Lafene's front desk or can be mailed to you for your convenience. It is often easier (and highly recommended) to have this form on file at Lafene before the student is injured or ill.

To obtain the form by mail, send an e-mail to lafene@k-state.edu. Type Medical History Form Request in the subject area and be sure to include name and mailing address in the body of the message. Return the completed form to Lafene Health Center: Attention - H.I.M. (Health Information Management department).

Release of information:

Only non-confidential information can be released without the written authorization of the patient. Information which can be released includes:

  • Name
  • Age
  • Birth date
  • Address
  • Verification of care (dates and times of admission and dismissal)
  • Attending physician
  • Immunization dates (may be released to patient only)

All other information requires that an Authorization for Disclosure of Protected Health Information [pdf] be completed and signed by the patient. Completed and signed forms may be faxed to 785.532.3425 according to Lafene's Fax Policy.

Tuberculosis (TB) testing:

Tuberculosis screening via a skin test (and x-ray when indicated), will be required prior to enrollment for all students who have spent more than four months in a country with a high incidence of TB. If you are uncertain if you need to be tested, please contact Lafene for assistance. Testing is encouraged (but not required) if the student has spent less than four months in a high-risk country. Any persons who accompanied the student to a high-risk area (e.g. spouse) are encouraged to be tested.

Vaccine recommendations

The Advisory Committee on Immunization Practices has made the following immunization recommendations for young adults. Be sure that your vaccinations are up to date. These vaccinations can be obtained from your local physician, health department or through Lafene Health Center's Allergy & Immunization Clinic.

  1. Measles, Mumps, Rubella: All entering college students born after 1957 should have had two vaccinations with the combination vaccine for these diseases (MMR vaccine). There are individual vaccines for each of these diseases. If individual vaccinations were administered, two individual doses of the measles vaccine, one of the mumps vaccine, and one of the rubella vaccine should have been given.
  2. The first dose of the MMR or measles vaccine should not be counted if you were younger than 12 months when you received it.

  3. Tetanus-Diphtheria: After the initial series, a booster is needed every ten years.

  4. Polio: This vaccine is routinely given to all infants. A booster may be needed for foreign travel after age 18.

  5. Varicella: Chickenpox can be a severe (even fatal) disease in adults. If you have not had the disease, this vaccination is recommended (two injections one month apart). Some persons are immune even though they are not aware they had the disease. If you prefer to see if you are immune, a blood test is available. Persons with a negative result should be vaccinated.

  6. Meningitis: Neisseria Meningitidis causes a rare but severe and frequently fatal disease called Meningitis. (Meningitis information for students/parents/guardians.) While this disease is no more common in college students than in the general population (about 1 in 100,000 students), it is five times more common in freshmen that live in residence halls. (1 in 20,000 students).
  7. Lafene Health Center can administer a vaccine that offers protection from the Neisseria Meningitidis bacteria. While the risk of contracting this disease is low, it can be rapidly fatal if contracted. This vaccine is recommended if your student will be a freshman living in the residence halls. The Meningococcal Vaccine is available at Lafene Health Center's Allergy and Immunization Clinic. Call Pharmacy at 785.532.7758 for current cost.

    "It is the policy of the Board of Regents of the State of Kansas that incoming students residing in student housing at a Regents' institution be vaccinated for meningitis or sign a waiver refusing receipt of the meningitis vaccine. Kansas State University is a Regents institution and is subject to the above policy." (from Housing and Dining Services Contract)

  8. Hepatitis B: This disease can be acutely fatal, or cause liver cancer many years later. Many students have already been vaccinated for this disease. This vaccine is recommended for all college students who have not had the disease or prior immunization. It is spread through blood, blood products, injection drug use (sharing needles) or sexual contact.

  9. Flu shots: Flu shots are recommended for anyone with a respiratory or other medical condition that could compromise their ability to fight infection. Sometimes these other conditions are exacerbated by lack of adequate hours of sleep and poor nutritional intake - lots of college students have both of these factors!!!
  10. Cost and availability information will be made known through Lafene Health Center's Allergy and Immunization Clinic beginning in late October.
